Note: *Any submitted project that is cancelled for any reason will require written notification to Intertek. *All material requiring molding must be accompanied by molding and drying parameters as well as M.S.D.S. sheets. Failure to provide these documents could result in a delay in testing turnaround. *Please clearly and legibly mark all containers that are to remain "sealed" until ready to test. *Return shipment of hazardous or DOT-regulated materials shall have M.S.D.S. and return shipment labels enclosed for prompt return (where applicable). *Client should provide detailed sketch if specific sample machining or preparation is required. Intertek shall not be responsible for any damages resulting from such procedures if proper documentation is not provided. *THERE IS A $200.00 MINIMUM INVOICE CHARGE. *TO ENSURE TIMELY PROCESSING - PLEASE FILL THIS FORM OUT COMPLETELY AND INCLUDE A PO NUMBER.
